We want to make sure you receive the scholarship you earned as quickly as possible, so please check your documents against the requirements before you submit to avoid delays in processing. We can ONLY accept PDFs of actual documents - we cannot accept jpegs, screen shots, or email attachments. Information must be clear/readable and contain identifying features such as your name, ID numbers, or address. We will let you know what is needed in the steps below. You will have the opportunity to add additional competition years. List the scholarship(s) you earned and their value (ex: Miss Congeniality, $100). If you have already redeemed one or more of the awards in a previous request, please include "PAID" next to the scholarship (ex. Miss Congeniality, $100, PAID) If the funds are older than a year you will need to upload all related extension requests.
