Please be aware that this process requires complete and correct documentation. We want to make sure you receive the scholarship you earned as quickly as possible, so please check your documents against the requirements before you submit to avoid delays in processing.

• We can ONLY accept PDFs of actual documents – we cannot accept jpegs, screen shots, or email attachments.
• Information must be clear/readable and contain identifying features such as your name, ID numbers, or address. We will let you know what is needed in the steps below.
• Many documents need to be recent (the last 30 days) and a date is required. We will let you know what is needed in the steps below.
• All information requested in this form is required by Miss America or our scholarship partner, Choice for All, to properly process your scholarship request.

Scholarship Information

1. Select the competition year where you earned the scholarships. You will have the opportunity to add additional competition years.
2. List the scholarship(s) you earned and their value (ex: Miss Congeniality, $100). If you have already redeemed one or more of the awards in a previous request, please include "PAID" next to the scholarship (ex. Miss Congeniality, $100, PAID).
3. If the funds are older than a year you will need to upload all related extension requests. (REQ: PDF, The awards you applied to extend, the date requested).
4. Repeat steps 1-3 for any additional competition years you are requesting.
5. Upload a letter of exhausted funds for all local competitions you entered and earned scholarship money for. You must either use or forfeit all local funds available to you before you can use money earned in the State Competition. (REQ: PDF, First and Last Name, Name of the Org, Year you earned the scholarships, Name and Value of scholarship, Local Director Signature).
6. Enter the amount of the funds you are requesting. note, this cannot exceed the amount you have earned.

Please be aware that not all funds can be used for all expenses. Please refer to the scholarship rules and regulations you signed as part of your state contract for information on what your scholarships can cover. Make sure you are referencing the scholarship rules that correspond to the year your scholarships were earned. Our scholarship rules evolve each year as we try to expand what can be covered. Document Requirements for student loan payment:

1. Proof that coursework is complete - this can be a copy of your degree or a copy of your transcript with your graduation date
2. A promissory note indicating that you are the primary payer on your student loans. This can be found online in your student loan account documents
3. A copy of your most recent bill, which must include your name, account number, the balance due (cannot be less than the amount requested), and is no older than 30 days

Amount requested for academic expenses

Documents required for reimbursement on academic expenses. We strongly recommend getting confirmation of coverage before planning to use scholarship funds for these expenses:

1. A copy of your course syllabus or a letter from a program administrator on school letterhead listing the books or equipment as required. Use of scholarship funding for technology and equipment is limited to $2,000
2. An itemized receipt no more than 60 days old. Only the items on the receipt which fit the above requirements and Miss America Guidelines for Scholarship Use will be reimbursed.
